Payment Policies

 Our office opperates on a fee-for-service model. Payments are expected at the time of service. We accept cash, check, Visa, or MasterCard.

Although we do not accept insurance for office visits, we will gladly provide appropriate forms  to submit to your insurance company for reimbursement. 

Missed appointments (without cancellation 24 hours prior to appointment) will be charged at the full rate.

Returned checks are subject to a $25 fee.
